I have not received a Dog License for my dog, although I’ve paid for it last Feb. 21 and the check was cashed by you 3/6/1998.

I paid $23.00 with check # 3798 for a 36-month Dog license (altered) for my and my wife ’s dog, Holly. A copy of the cancelled check is enclosed and a copy of the dog license application.

Please send a dog license tag or refund my $23.00 (to the Address above). Thanks.
