Dear Sir or Madam:

Please consider this my formal claim against the County of San Diego for either $23.00 or the license I paid for. If I do not receive a response in 30 days, I will consider the claim denied and will file an action in small claims court to recover my money.

Here’s some background. I paid $23 for a 36-month license for an altered dog. The check was cashed 3/6/1998, but I wasn’t issued a license. My follow-up letter has been ignored, so I’m writing this letter as a last-ditch effort before taking this matter up in court. Enclosed is a copy of the cancelled check, dog license application, and the follow-up letter.

Please respond and save us both a lot of time and expense. Thanks!
